An exciting opportunity awaits a Support Coordinator to join our team in Dandenong.

Responsibilities 

As an Support Coordinator at Vision Australia you will:

  • Use a person centred, strength-based service approach to plan, develop and coordinate services with clients through the initial contact process and liaise with internal and external services to maximise client outcomes; 
  • As required, act as the Primary Service Provider (PSP) to coordinate the provision of services internally for clients and families and to liaise with external service providers to achieve desired outcomes for clients.
  • Advocate for and mentor participants of NDIS, My Aged Care and other individualised funding schemes;
  • Work collaboratively and proactively with service delivery staff, Service Engagement Consultants, Regional Practice Leaders and Regional Business Managers to provide transdisciplinary, holistic services for Vision Australia clients

Requirements

To be successful in this role you will have:

  • Tertiary qualification in Health Sciences, Community Development, Social Work, Allied Health or equivalent;
  • Demonstrated experience in working with people of all ages with disabilities in the community setting and providing client centred assessment and service delivery;
  • Knowledge and experience of funding programs and working to target. This includes negotiation, issue resolution and relationship development;
  • Experience in advocating for options and services to holistically meet an individual’s needs
