An exciting opportunity awaits an experienced Home care Packages Case Manager to join our team in Dandenong.

Responsibilities 

As a Home Care Packages Case Manager at Vision Australia you will:

  • Using a person centred, strength-based service approach, plan develop and coordinate services for the client through the intake assessment process and liaise with internal and external services to achieve the desired outcomes for clients;
  • Monitor and review service plans and goals in consultation with clients on an ongoing basis. Provide support and assistance as appropriate to relevant parties;
  • As required, act as the Primary Service Provider to coordinate the provision of services internally to clients and families and to liaise with external service providers to achieve desired outcomes for clients. 

Requirements

To be successful in this role you will have:

  • Degree in relevant Human Services or Diploma in relevant Human Services studies with a minimum of 2 years recent experience in the delivery of Home Care Packages.
  • Knowledge and skills in working with older adults their carers and families.
  • Experience in providing client support and Case Management services in person and/or through other communication mediums.
  • Training and intervention strategies for individuals and groups.
